Manager/ Supervisor to advise CHSL
- What the injury is
- Where the injury occurred
- How the injury occurred
- The current health status
- Where the injured employee is currently
- Current availability of light duties and return to work requests

Employee will be advised to attend one of the following
- Remain on-site for First Aid treatment
- Accident & Emergency at the Hospital, Maunu
- White Cross Medical Centre, Whangarei
- Minor Treatment, Company Medic

Nurse will advise Health Provider of the above information and attend to manage injury within clinic if practicable. Recommend light duties and advise on return to work availability for company.
Treatment will be Provided and ACC claim submitted

Following informed consent from employee - CHSL requests treatment information, ACC claim details and diagnosis from GP or health provider

Provide written report and/or verbal feedback to company with
- Details of injury
- Return to work procedures
- Rehabilitation Plan & next review date
- Referral to other agencies or health providers required
